This Change Management Model can describe the situation of my company’s experience well.
The CASE is about migrating databases from old systems to a new ERP system, since there are 4 different databases to store clients, products, accessories and staff information. As all the data are separated and without linkage, our company would like to migrate all of them into a new interfaces’ ERP system to centralize all the databases and build up an integrated platform for easier management.
There are ten steps of this change:
1. Learning
- Understand the current situation of our own company and competitors, including strategies and systems.

2. Recognizing the need for change
- Compare the differences between our company and competitors and find out others’ successful factors.

3. Setting a vision for change
- Consolidate information from vendors and viewing their presentations for investigation and discussion.

4. Understanding culture and polities
- Get approval from Management and consider man power and other issues.

5. Promoting and employing leadership- Hire technical staffs and experienced consultant to help on running the project. Besides, promoting staff for handling the progress.
6. Identifying a change approach- Arrange meeting internally and announce the migration ideas to all colleagues.
7. Planning the change
- Arrange meeting with vendors and design the project plan, including timeline of first, second and third stages. (Testing -> In Progress -> Evaluate)

8. Implementing the change- Start testing and proceed the First stage – Testing and Implementation.
9. Controlling the change process- Second stage – Keep monitoring the progress and make sure the migration is smooth with no issues.
10. Evaluating change- Third stage – Evaluating the progress, and make amendment if needed.
After the tenth step, it will move it back to the first step - Learning so as to run as a cycle for further changes.

As leaders play important role in leading change, this step may contribute greatly to organization's success. Furthermore, the last step in this model helps maintain sustainable change, which provide continuous innovative ideas to organizational learning.
Good, yes, change in organisations must be continual and sustainable too.
